The Registration Application is divided into three (3) sections:
1. Company Information
Legal Name - The name under which your company was legally registered in your jurisdiction.
Type of Company - there are three choices of company: incorporated, partnership or sole proprietorship.
DBA - Is you company’s Dun & Bradstreet number, if you know it.
Fed. Tax ID or GST # - If you are in the United States, please indicate your federal tax identification number. If you are located in Canada, pleas include your GST number.
Principal Information - Company principals are the signing officers in a corporation, members of a partnership with signing authority, or the sole proprietor in the proprietorship. Please fill in the principals’ name, title and Social Insurance Number if the company is a partnership or sole proprietorship.
Billing Address - Is typically the registered address of the company.
Delivery Address - The location where you would like your pump sets delivered. Upon manufacture of your order, you will be notified when your unit is to ship. You must be able to receive your new pump set at this location within one week of date of shipment. Delivered will only be made to this location. Deliveries are not made to site locations.
Phone & Fax Numbers - Please indicate the company phone number, and the fax number where you wish your PUMPSETSDIRECT.COM quotation faxed (if so desired).
Provincial Sales Tax Exemption Number - If you are an Ontario registered company, please indicate your PST exemption number, if relevant.
2.Major Credit References
Company - Please list any companies with which you do business and have established credit.
City/Province(State)/POstal code (zip code) - Please list the addresses of the companies with which you have established credit.
Telephone/Fax Numbers - Of the companies with which you have established credit.
3. Bank References
Please provide the name, address and contact information of the bank with which you do business. Please also include your bank account number.

FORM 1 is divided into five (5) sections:
1. Quotation Information
Current Date - The date when you submitted the quotation.
Company ID# - This is the PUMPSETSDIRECT.COM confidential registration number for your company. This is the number boldly outlined on the confirmation reply sent after your registration.
Contact Name - This is the name of the contact person for the RFQ being submitted if other than the name of the person originally listed when you registered with PUMPSETSDIRECT.COM.
Phone Number - The phone number of the person in charge with this RFQ, if other than the one originally given to PUMPSETSDIRECT.COM when you initially registered.
Project Number - This is your project reference number given to this RFQ. If this is left blank, PUMPSETSDIRECT.COM will fill in a reference number.
2. Pump-Set General Requirements
“PUMPSETSDIRECT.COM to choose number of pumps based on lowest price” - Check this box if you would like to us to make the most economical selection based on your flow and pressure requirements. You may also set physical dimensions for the unit (see blow).
“Check to request standby” - Check if you wish us to include a standby pump with our selection above.
Overall Flow - Indicate the total discharge flow required from the pump set (in USgpm). Note, maximum flow is around 900 USgpm.
Required Pressure - Indicate the total discharge pressure for the tank set, for the flow indicated above.
3. Pump-Set Physical Limitations
Indicate the total maximum WIdth, Depth and Height for the complete pump set.
4. Pump-Set Skid Details
Pump System - Make the appropriate selection for you system based upon the following guidelines:
i.1 pump - 1 duty
ii.2 pumps - 1 duty and 1 standby
iii.2 pumps - 1 duty and 1 duty
iv.3 pumps - 1 duty, 1 duty and 1 standby
v.3 pumps - all 3 duty (operating)
NOTE: “duty” means that the pump is in service and “standby” means that the pump is in back-up mode, ready for service if needed. All pump sets come with common inlet headers and common discharge headers, standard.
Piping Material - The default material is steel (ferrous). If copper or 304 stainless steel are chosen, then the pump housings are stainless, but the impellers are bronze. Multistage pumps remain stainless with bronze impeller.
NOTE: If bronze impellers are a problem for your particular application, please note the conflict on your FORM 1. Other materials are available for custom applications at extra cost. Custom project may take longer to quote than 72 hours.
Voltage - Please chose your required voltage: 575, 460 or 208 Volts. All systems are 60 hertz.
Add expansion tank - You have the option to have an expansion tank mounted and plumbed directly into your pump set. The expansion tank is sized based on the discharge pipe diameter and a system volume of 200’ of piping. If your system is larger, please make a note of the total system volume for expansion tank sizing purposes.
5. Pump Selection Details
Inlet type - There are two selections available for each pump inlet:
i.Isolation valve, or
ii.Suction diffuser with isolation valve.
Pump type - There are three pump types to choose from:
i.Multistage,
ii.Inline, or
iii.Close-coupled.
Outlet type - There are three pump discharge combinations to choose from:
i.Check valve and isolation valve,
ii.Triple duty valve, or
iii.Balancing valve, check valve and isolation valve.
